3. COMPLETE THE ONLINE REGISTRATION PROCESS

• You can choose to upload a birth certificate, immunization records, and proof of residency during the registration process OR you can bring it to your appointment in step 4 below.

4. SCHEDULE AN APPOINTMENT WITH YOUR STUDENT'S SCHOOL

CPCSC will utilize an appointment system to help young students feel comfortable in a new setting.  Families should sign up for a time (links below) at the school their child will attend.  We ask that families limit the number of visitors attending registration to 2-3 people including the registering student. During the appointment time, you and your student will complete the following:

• Students will complete a kindergarten readiness assessment with a CPCSC staff member.

• Parent/Legal Guardian will need to turn in a birth certificate, updated immunization records, and proof of residency (utility bill) if you did not upload them during the online registration process. Please also provide any legal documentation (custody paperwork, guardianship agreements, restraining orders, etc) that is relevant to your student.
